IMPORTANT INFORMATION

This document affects your legal rights. By signing it you agree to assume all risks and waive the right to sue. You should read it carefully. No admission to the Skate Park or Cornerstone Games will be granted without prior completion of this document.

Cornerstone Festival holds the safety of participants in high regard and strives to reduce risks. All participants must follow safety rules and instructions. However, Cornerstone Festival does not take responsibility for supervising the activities of participants or spectators. Participants and parents/guardians of minors must recognize that skating, skateboarding, rollerblading, in-line skating, volleyball, basketball, soccer, canoeing and all other Cornerstone Games are hazardous recreational activities with a substantial risk of injury to participants, assistants and spectators.

You are solely responsible for determining if you or your minor child/ward is physically fit and adequately skilled for the activities at the skate park or Cornerstone Games.

WARNING OF RISK |
I acknowledge and understand that skating, skateboarding, rollerblading, in-line skating, volleyball, basketball, soccer, canoeing and all other Cornerstone Games are hazardous and dangerous activities that require strenuous exercise and various degrees of skill and experience for the different skating surfaces and Cornerstone Games venues. I understand that these activities have a substantial risk of serious injury to the participants and spectators and damage to property. I voluntarily assume all risks of loss, damage or injury while on the premises.

I understand that the risks, hazard and dangers associated with skating, skateboarding, rollerblading, in-line skating, volleyball, basketball, soccer, canoeing and all other Cornerstone Games include, but are not limited to, striking an irregularity in the riding surface, losing balance, trying to perform stunts, the skateboard slipping out from under the rider, tripping over a curb, loss of control, colliding with other participants or stationary objects, acts of God, inclement weather, lack of safety equipment, inadequate or defective equipment, failure in supervision or instruction, premises defects, attempting a maneuver beyond one's skill level, and all other circumstances inherent in skating or participating in Cornerstone Games. In this regard, I recognize that it is impossible for Cornerstone Festival to guarantee safety.

WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK |
Please read this form carefully. By participating in activity at this skate park or the Cornerstone Games, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with activity at this skate park or Cornerstone Games venue.

I recognize and acknowledge that there are substantial risks of serious physical injury associated with activities at this skate park or Cornerstone Games venue, and I voluntarily agree to assume the full risk of any injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of participating in any activities, assisting or spectating at the skate park or Cornerstone Games venue. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in this activity against Cornerstone Festival, Jesus People USA FGM, Jesus People USA Covenant Church, including its officials, agents, volunteers and employees.

I hereby release, waive, discharge, and covenant to not sue Cornerstone Festival, Jesus People USA FGM, Jesus People USA Covenant Church, their affiliates, officers, member, and staff, including volunteers (individually and collectively, "Releasees" on my own behalf and on behalf of my personal representatives, heirs, and next of kin, from any and all claims for injuries, damages or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with activity at this skate park or Cornerstone Games. I further agree to indemnify the Releasees and each of them from any loss, liability, damage or cost Releasees may incur due to my participation in the skate park or Cornerstone Games.

Use of safety equipment is strongly recommended at the skate park. Helmets are required for participants under age 18. Please check the line below if you are 18 or older and do not wish to use a helmet:
